2025 3 on 3 Basketball Tournament Registration Form - Women's Age 13-15

Each year, the 3 on 3 Basketball Tournament at the Elkhart County 4-H Fair is one of the most popular events at the fair. Hundreds of men, women, and children compete in the contest held on the Fair’s outdoor courts. There will be different age divisions playing each night Monday, July 21 through Thursday, July 24.

New format: guaranteed a minimum of 2 games.

An entry fee of $50 per team must accompany the entry form. Entries are limited to 5 for this division. We will accept entries until Saturday, July 12, 2025, 12 pm unless all slots are filled prior.

Fair Admission Tickets need to be purchased by all first-round competitors. Advance Sale Admission Tickets are available until the week before the Fair at a discounted price. Complimentary tickets are given to the teams that make it to the finals. Rosters may include four players.

A waiver/release form must be signed by all competitors. If the competitor is under the age of 18, the release form must be signed by a parent. Participants will not be able to play until release forms are signed. Please upload a copy of your signed waivers with this form or bring signed waiver forms to check in at the basketball courts 30 minutes prior to your first scheduled game.
